I switched to Build Previews: Standard, because embedded previews have a couple of known bugs, and some recent measurements suggest they're no longer faster than just using standard previews for the initial import. So the rule is: Dont rotate a photo when it is showing the double-arrow embedded-preview badge. You can also annotate an existing image in your library by clicking on the more button upon item hover in the listing. Capture a screenshot using the camera icon on the top bar or simply using the alt+shift+1 shortcut (for windows).

I have no idea why and it's super annoying.
